What does “normal” testosterone actually mean?
My results were inside the green band, but closer to the lower end. That led me to look more closely at what a testosterone reference range tells us, what it leaves unanswered, and when the conversation belongs with a physician.
On June 26, my testosterone results came back within the laboratory's normal ranges. My total testosterone was 456 ng/dL, with a reference range of 300 to 1,080. My calculated free testosterone was 78.8 pg/mL, within a range of 47 to 244. My sex hormone-binding globulin, or SHBG, was 27 nmol/L, within a range of 17 to 56.
My physician reviewed the results and found no health concern. Still, when I opened the report, I found myself looking less at the word “normal” and more at where the markers appeared inside the green bands. Both testosterone measurements were closer to the lower ends of their ranges than the upper ends.
Testosterone is a common subject when you work in fitness. Clients ask about its relationship to muscle growth, body composition, energy, recovery, sexual health, and aging. I can provide general education about those subjects, but interpreting an individual laboratory result, diagnosing a hormone condition, or advising someone about treatment belongs with a qualified medical professional. When a client wants to move beyond general information, I refer that conversation to a physician.
This time, however, the results were mine, and I became the person with the questions.
Was the green band showing me a target, an average, or simply the boundaries of a reference interval? Did a result farther to the right indicate better health or greater physical potential? Did being closer to the left mean that something was beginning to go wrong, even though the result remained normal?
Those questions have become especially timely. In July, Pete Hegseth directed that active-duty and reserve service members age 30 and older be screened for testosterone deficiency as part of their Periodic Health Assessments. The memorandum connected the policy to military readiness and human-performance optimization, while allowing younger service members to request screening.
When testosterone is being discussed at that level, understanding what happens after the blood draw becomes important. A test can produce a number in minutes. Knowing what the number means requires much more context.
The green band is not a performance scale
A reference range is the span of results a laboratory uses to help interpret a test. These ranges are generally developed from measurements taken from groups of healthy people, and laboratories may use different methods, populations, and ranges. That is why the reference interval printed on the actual report matters more than a range found through an online search. Results from different laboratories may not be directly comparable.
The way results are displayed can still encourage the wrong interpretation. A horizontal bar has a beginning, a midpoint, and an end, so it is natural to read it like a score. The left side looks worse, the right side looks better, and the upper boundary begins to resemble a goal.
That is not what the laboratory is communicating.
A testosterone result of 900 is not automatically three times “better” than a result of 300. The midpoint of the range is not a minimum standard for an athlete, and the upper limit is not a target every healthy man should pursue. The band is helping a clinician compare the result with the laboratory's reference population. It is not ranking strength, muscularity, masculinity, energy, or overall health.
The word “normal” can also create too much certainty in either direction. MedlinePlus notes that healthy people can occasionally have results outside a reference interval, while people with health conditions can sometimes have results that fall within it. The number is one piece of the clinical picture rather than a stand-alone verdict.
Total testosterone is only one part of the panel
Most people hear their total testosterone number first, but total testosterone includes both the hormone that is bound to proteins and the smaller portion that remains unbound, or free.
Most circulating testosterone is attached to proteins such as SHBG and albumin. Free testosterone is the unbound portion that is more readily available to tissues. A total testosterone test measures the combined amount, while free or bioavailable testosterone testing may provide additional information in some clinical situations.
SHBG helps place the total number in context because it affects how much testosterone is bound. When SHBG is unusually high, less of the total testosterone may be readily available to tissues. When it is unusually low, a larger portion may remain available. Clinicians commonly consider SHBG together with total testosterone when estimating or evaluating free testosterone.
My report included all three:
- Total testosterone: 456 ng/dL
- Calculated free testosterone: 78.8 pg/mL
- SHBG: 27 nmol/L
All three were within the ranges reported by that laboratory. My calculated free result did not reveal an abnormal value hidden behind an in-range total result, and my SHBG was not flagged as unusually high or low. That helps explain why my physician viewed the panel as reassuring.
It does not mean everyone with similar numbers will receive an identical interpretation. Age, symptoms, medical history, medications, the testing method, and the reason the test was ordered can all influence what a clinician considers relevant.
“Lower end of normal” is a description, not a diagnosis
It is reasonable to describe a result as being toward the lower end of a laboratory range. The phrase becomes misleading when “lower-normal” is treated as a separate medical condition.
Clinical guidelines do not diagnose testosterone deficiency according to the position of a marker inside a green bar. The Endocrine Society recommends diagnosing hypogonadism when a man has symptoms or signs consistent with testosterone deficiency together with testosterone levels that are unequivocally and consistently low. It recommends confirming a low value with a repeat morning fasting measurement.
The American Urological Association considers a total testosterone result below 300 ng/dL a reasonable cutoff in support of a diagnosis, but the number is still evaluated alongside symptoms and other clinical information.
That distinction helped me understand my physician's response. My total testosterone was 456 ng/dL, my calculated free testosterone was within the laboratory range, and he found no health concern. The fact that I could see more green space to the right of the markers did not create a diagnosis that was absent from the clinical evaluation.
The reverse is also important. A person with persistent symptoms should not assume that an in-range result automatically ends the conversation. The appropriate next step is to discuss those symptoms and the complete medical picture with the clinician, not to reinterpret the bar independently.
One measurement has limits
Testosterone is not completely static. Levels are typically highest in the morning, which is why blood testing is commonly performed between 7 and 10 a.m. In some circumstances, fasting or changes to medications may also be relevant, depending on the physician's instructions.
The Endocrine Society's current position is that an accurate diagnosis requires symptoms plus consistently low and accurately measured total or free testosterone. It recommends at least two early-morning fasting tests when evaluating possible hypogonadism and warns that nonstandardized assays can sometimes cause the same sample to be classified differently depending on the laboratory or method.
That does not mean every normal result needs to be repeatedly tested. It means a single value should be interpreted according to why it was ordered and what the physician is evaluating. A result used as part of a routine assessment is not necessarily being asked to answer the same question as testing ordered because of persistent symptoms, fertility concerns, medication effects, or a known endocrine condition.
The patient portal tends to flatten all of that context into one number and one color. The physician's interpretation restores the information the graphic cannot display.
Why the military screening debate matters
The new military directive places testosterone in a much larger public conversation about health, readiness, and performance. Its language links deficiency screening with human-performance optimization and requires screening for a broad group of service members, not only those who have already reported symptoms.
The following day, the Endocrine Society issued a statement emphasizing accurate diagnosis and stating that there is insufficient evidence to recommend population-level screening for hypogonadism among asymptomatic men. The organization reiterated that symptoms, consistently low results, testing quality, and clinical judgment remain central to diagnosis.
Those positions reveal an important question that extends beyond the military: What is screening intended to accomplish?
Screening may identify people who deserve further clinical evaluation. It does not turn testosterone into a readiness score, and it cannot establish from one measurement who is stronger, more capable, or performing closer to his potential.
This becomes particularly important when the language of deficiency and the language of optimization begin to overlap. A test designed to help identify a medical condition is different from a test used to ask whether someone might achieve more muscle, greater energy, or better performance at a different level.
The same laboratory number can enter both conversations, but the questions are not interchangeable.
What should you ask when your result is near the lower end?
A result inside the green band may be reassuring, confusing, or both. Rather than comparing the marker with the top of the range, it is more useful to begin with the context in which it was measured.
- What reference interval did this laboratory use? Different laboratories and testing methods can produce different ranges, so the report should be interpreted using the interval provided with that test.
- Why was the test ordered? A routine health assessment, persistent symptoms, fertility concerns, medication monitoring, and follow-up of an earlier result are different clinical situations.
- What exactly was measured? Total testosterone, free testosterone, bioavailable testosterone, and SHBG describe related but different parts of the picture. One number should not be substituted casually for another.
- Were the timing and testing conditions appropriate for the question being asked? When a physician is evaluating possible testosterone deficiency, morning collection and repeat testing may matter.
- Are there symptoms or health changes that need further evaluation? Low energy, reduced libido, mood changes, loss of muscle, and other commonly discussed symptoms can have multiple causes. They deserve a clinical assessment rather than an assumption that testosterone must be responsible.
These are questions to bring into a medical conversation. They are not a formula for diagnosing yourself.
Where a trainer's role ends
A fitness professional should understand why testosterone comes up in conversations about muscle, bone, aging, recovery, and body composition. General education can help clients ask better questions and recognize when a claim online is oversimplifying the subject.
That education should not drift into personal medical interpretation. A trainer should not tell a client that an in-range result is secretly too low, that a symptom proves a hormone condition, or that treatment is necessary. Those conclusions require medical training, a full clinical history, appropriate testing, and an individual risk-benefit discussion.
Staying within scope does not mean refusing to discuss testosterone. It means being clear about the difference between explaining a concept and evaluating a patient.
My results gave me a reason to study that distinction more closely. I accepted my physician's conclusion, but I also wanted to understand why a normal result could still look incomplete when displayed on a wide green bar.
What the green band finally meant to me
My results did not identify a health concern. They did reveal how easy it is to ask a reference interval to tell us more than it can.
The green band showed that my results fell within the ranges used by that laboratory. It did not show where I should aspire to be, what another man's result meant for me, or how much muscle, energy, or physical capacity any point along the bar should produce.
A result near the lower end of normal may prompt reasonable questions. The useful response is not to assume that it represents either a hidden problem or a complete assurance of health. The useful response is to understand what was measured, consider why it was measured, and discuss the result with the clinician who has the context to interpret it.
Testosterone will continue to be a major subject in fitness, medicine, military readiness, and the wider conversation about aging and performance. As more people receive tests, more people will open their portals and see a number somewhere inside a green band.
They deserve to know that the band is a reference, not a ranking.
And in fitness, knowing the difference between those two things is part of helping people make informed decisions without stepping beyond the role of a coach.
